INFORMATION SHEET FOR PENNSYLVANIA CONSUMERS

This information sheet contains important information about your rights and our responsibilities under the Pennsylvania Credit Services Act.

In accordance with the Pennsylvania Credit Services Act:

1. you have the right to review and/or receive a copy of any of your files maintained by any consumer credit reporting agency.

2. the approximate cost for your receipt of any of your credit files is Nine Dollars ($9.00) plus tax.

3. the information contained in your credit files will be provided to you free of charge by the consumer credit reporting agency by telephone after written request within thirty (30) days of you receiving a denial of credit notice.

4. you have the right to dispute the completeness or accuracy of any item contained in your credit file maintained by any consumer credit reporting agency.

You acknowledge and understand that The Lasik Vision Institute will not be charging you any amounts in connection with any efforts that you may make to finance the cost of any surgery that is performed at a facility owned or operated by The Lasik Vision Institute.  You acknowledge and understand that The Lasik Vision Institute is not a consumer credit reporting agency as that term is defined in the Pennsylvania Credit Services Act.
